Icon Legend


The following tables provide a reference list for any and all icons used in Vault.

Query Statuses

Queries in Vault have three statuses: Open, Answered, and Closed. These statuses indicate which actions have been taken and what still needs to occur on a query.

Status Icon Meaning
Open Open Query Badge Open indicates that a query has not been answered. The Open Query badge icon displays in the casebook schedule on the queried event or form's existing status icon. Vault lists open queries in the task bar.
Answered Answered Query Badge Answered indicates that a query has been addressed. For example, the site has responded with a reason for the queried value. The Answered Query badge icon appears in the casebook schedule on the queried Event or Form’s existing status icon.
Closed N/A Closed indicates that a query requires no further action or discussion. A CRA or Data Manager can reopen a closed query if needed.

Event Status Icons

Events in the casebook have status indicator icons. The table below orders icons by priority, from highest to lowest. For example, the Overdue status will appear on an Event in the casebook schedule, even if the Event also has the Open Query status. If you hover your mouse over any icon in the application, Vault EDC displays text with the icon’s meaning.

Overdue Icon Overdue The Event’s due date has passed and at least one form has the Planned status.
Open Query Badge Icon Open Query At least one Form in the Event has an Open Query (at the form- or item-level). Vault EDC adds an Open Query badge to the Event status icon.
In Progress Icon In Progress The Event has an Actual Start Date, and at least one form is not Submitted or has at least one task. This status can also indicate that at least one but not all Forms in the Event are frozen or locked.
Submitted Icon Submitted All Forms in the Event are submitted and no forms have a Planned or Blank status.
Blank Icon Blank The Event has an Actual Start Date, but all forms are in the Planned status.
Planned Icon Planned The Event has no Actual Start Date.
Did Not Occur Icon Did Not Occur The Event is marked as Did Not Occur.

Form Status Icons

The Forms displayed in the casebook schedule also have status indicator icons. The table below orders icons by priority, from highest to lowest. If you hover your mouse over any icon in the application, Vault EDC displays text with the icon’s meaning.

Overdue Icon Overdue The Form is in Planned status and the Event due date has passed.
Open Query Badged Icon Open Query There is at least one Open Query on an Item in the Form. Vault EDC adds an Open Query badge to the Form status icon.
Submitted Icon Submitted The Form has been submitted and there are no open tasks.
In Progress Icon In Progress The Form has not been submitted since the last data change or there are tasks not completed on the Form. 
Blank Icon Blank The Form has not been submitted, and has no queries, autosaves, or fields left blank.
Intentionally Left Blank Icon Intentionally Left Blank A data entry user marked this Form as Intentionally Left Blank.
Locked Icon Locked All Items in the Form are Locked.
Frozen Icon Frozen All Items in the Form are Frozen.

Task Icons

Each task has an icon to indicate its task type.

Icon Meaning
Overdue Forms Icon Indicates that a form is overdue.
In Progress Forms Icon Indicates in progress, incomplete forms.
Open SDV Task Icon Indicates a required source data verification task.
Open DMR Task Icon Indicates a required data management review task.
Signatures Required Icon Indicates that a submitted form requires your eSignature.
Open Queries Task Icon Indicates that a form has open queries.
Answered Queries Task Icon Indicates that a form has answered queries.

SDV & DMR Icons

Vault uses these icons to represent SDV and DMR review statuses:

SDV Optional Icon DMR Optional Icon An unchecked optional DMR or SDV icon indicates that SDV or DMR is optional, but is not complete.
SDV Optional & Complete Icon DMR Optional & Complete Icon A checked optional SDV or DMR icon indicates that DMR or SDV is optional and complete
SDV Required Icon SDV Required Icon An unchecked icon indicates that SDV or DMR is required.
SDV Required & Complete Icon DMR Required & Complete Icon A checked icon indicates that SDV or DMR is required and complete.
Review Partially Complete Icon Review Partially Complete Icon This icon indicates that SDV or DMR is partially complete. This status rolls up to the Event level. If an Event has three Forms, and only one of those Forms has DMR completed, the Event displays this icon, and the Form displays the DMR complete icon.
No Icon No Icon No icon indicates that SDV or DMR is not required.

Form Visibility Icons

The following icons represent the visibility selection:

  • All Items on the Form are visible.
  • The Item is visible.
Some Items are Visible icon
  • Some Items on the Form are visible, but at least one Item is hidden.
Hidden icon
  • No Items on the Form are visible.
  • The Item is not visible.
